Client releases from all actions, causes of action, suits, claims and demands whatever against elizabeth permanent cosmetics, that may arise from any and all damages and injuries client may incur as a result of the services performed and all materials used and furnished to me in connection with the application of products furnished to me or applied to any part of my face or body by the technician.
In giving my consent to this procedure, I declare that I understand the following:
* that as with all such procedures, micropigmentation is not an exact science, and I acknowledge that it only attempts to improve enhance accentuate and beautify, not replace ordinary make-up.
* that the outcome in my case is directly related to my specific body, skin nature and condition. You should seek the advice of a medical doctor if you have any questions. I acknowledge that elizabeth roman does not give medical advice.
* that some complications can occur: allergic reaction, infections, persistent bruising, redness.
* that the treated area will be swollen and bruised and somewhat sore following the procedure.
